Employment Status: Temporary Full-Time Contract (3 months)
Work Hours: Monday – Friday, 35 Hours per week (1 evening per week)
Location: Pickering, ON; (with flexibility to transition to/work at other sites as required).
We are seeking a dedicated Primary Care Nurse Practitioner to join our healthcare team. The ideal candidate will have experience in various medical settings and be passionate about providing comprehensive care to patients of all ages.
Job Duties:
- Provide client-centered primary care, addressing the social determinants of health and advocating for clients.
- Participate in inter-professional care, consulting with NPs, physicians, nurses, therapists, and external health service providers.
- Perform clinical assessments, diagnostics, and treatment within the full NP scope of practice, including prescribing narcotics and controlled substances.
- Lead and collaborate effectively within a team, utilizing strong communication and interpersonal skills.
- Manage conflict with evidence-based solutions.
- Offer on-call coverage as needed.
- Address clients’ biopsychosocial needs, including those with comorbid conditions, mental health issues, and substance use disorders.
- Provide counseling, education, health promotion, referrals, and follow-up care.
- Engage in community development, outreach, and resource management with partners and stakeholders.
- Manage client treatment regimes, maintain accurate medical records, and contribute to the development and evaluation of medical directives and policies.
- Advocate for at-risk clients and participate in case conferences, chart reviews, Quality Improvement Plans (QIP), and accreditation activities.
- Ensure continuing competence through CNO Quality Assurance (QA) Program participation and agency performance management.
- Participate in training, meetings, and home visits for community clients.
- Provide clinical supervision for NP students and perform additional duties within the scope of practice.
- Flexible to work or attend meetings at various locations within the service area.
Qualifications:
- Master’s degree in Nursing (MSN) and completion of a recognized primary care NP program (NP-PHC).
- Certification with the College of Nurses of Ontario (CNO) as RN (Extended Class); additional membership with NPAO or RNAO is an asset.
- Proficiency in PS Suite
- Minimum of three years’ experience as a primary care NP, particularly with complex cases and children/youth; experience in community health settings is beneficial.
- Strong critical thinking, problem-solving, communication, and organizational skills; ability to work effectively in a collaborative team and manage crises professionally.
- Valid CPR certification, current vulnerable sector police check, Ontario Driver’s license, and access to a vehicle; proficiency in Microsoft Office and experience in client group activities are assets.
